Online Application
for Class 2 Individual Certificate
Guidelines
- Class
2 Individual certificate is accepted at MCA 21, NSDL, Income Tax etc.
- Applicant
should fill the online form first and then download the hard copy of
the application form by clicking the link “DSC Application form
and checklist for class 2 Individual “ in the following web page
http://mtnltrustline.com/repository/forms/index.html
.
- Completely
filled and duly signed application form in hard copy is to be sent to
the nearest Physical RA within 60 days of online application. Physical
RA address is given at the end.
- Along
with the completely filled and duly signed application form, please
attach copies of three documents as mentioned below:
1.
Identity Proof (As selected in the online form)
Attested
copy of any one of the following: PAN card/ Voter Id Card /Passport
/Ration Card/ Bank Account Details/ Driving License. [Attestation should
be done by a gazetted officer
/CA/CS/Bank attestation(from nationalized banks only)]
2.
Proof of address:
Self
attested copy of any one of latest utility bill (water, electricity,
power, or telephone) in the applicant’s name or ration card/bank
statement/ latest credit card bill in the applicant’s name.
3.
Photo copy of ID card of attesting officer.
Also
send the DD with the application form if payment is made through demand
draft (write Pre-Registration Number, Name, Phone Number on back side
of DD).

can be made either by

Demand Draft (Send it along with the application form)
Demand
draft should be drawn in favour of 'MTNL Delhi' if application form
is send to Physical RA Delhi, OR in favour of ‘MTNL
Mumbai’ if application form is send to Physical RA Mumbai.

Note:
- After
successful pre-registration, you will receive an e-mail with
pre-registration number which is required for all future
correspondence.
- Quote
this pre-registration number at the top right hand corner of every
page of the filled hard copy application form and other documents.
- Superscribe
the envelope in which the application form is send with “Application
for MTNL Class 2 Individual Certificate - Pre Registration Number”.(e.g.“Application for MTNL Class 2 Individual Certificate – 100019”).
- Once the complete form with all the supporting documents (as
selected in the online form) is received, it will be accepted and an e-mail will be
send with password to claim the certificate.
- If the form is not complete or documents are not proper, the application will be rejected and an e-mail is send with reason(s) for rejection. .
