1. Proof for
organization name and Authorisation letter (regarding person and
e-mail Id): d. For organizations created by an Act of the Parliament - A copy of the relevant Act or Government Gazette plus an attestation of the application form by an officer of the rank of Head of Department or above. e. For government departments - An attestation of the application form by a gazetted officer of the rank of Joint Secretary or above. f. For any other organization like a NGO, etc - Certificate issued by appropriate Government or Government Agency. 2. Proof of right to use domain name is required only in case the domain-name is not owned by the organization applying for the certificate. This is in the form of an “Authorization of Domain Name Use” letter from the organization owning the domain name. This letter has to be on the letter head of the organization owning the domain name and must be signed by the person listed as the organizational contact in the “DNS whois” records for that domain. The template for the letter is available at https://www.mtnltrustline.com/repository/adnu.html or can be obtained from the RA. The proof of possession of the appliances is to be submitted. 3. Copy of E-mail
Received. Enrollment for Class 3 Individual Certificates is through the physical RA. The applicant fills an Application Form. Paper copy of the application form is available with the RA; a soft copy of the form can also be downloaded (for printing) by the applicant from mtnlTrustLine’s web-site at <https://mtnltrustline.com>. Along with the completed and signed application form, the applicant is required to attach self attested copies of three documents as mentioned below: Document 1, Photo ID Card with applicant’s signature on it - Self attested copy of any one of Passport or PAN Card. The document should be valid at the time of certificate application Document 2, Second Photo ID Card - Self attested copy of any one of Passport, PAN Card, Voter’s ID card, Driving License, Photo Credit Card, or Photo Id Card from employer. This document should be different from the one submitted above and should be valid at the time of certificate application. Document 3, Proof of address - Self attested copy of any one of latest utility bill (water, electricity, power, or telephone) in the applicant’s name, or latest credit card bill in the applicant’s name. The applicant is also required to bring the originals of the above two photo ID cards for verification at the time of certificate application. The applicant is also
required to sign the mtnlTrustLine Subscriber Agreement attached to the
application form. Application for Class 3 Server Certificates is through the physical RA. The applicant fills an Application Form. Paper copy of the application form is available with the RA; a soft copy of the form can also be downloaded (for printing) by the applicant from mtnlTrustLine’s web-site at <https://mtnlTrustLine.>. The applicant is required to provide the following minimum information:
The applicant submits the enrollment form at an RA location. A list of the current RA locations is provided at https://www.mtnltrustline.com/repository/ra-list.html. The application form should be attested (signed and stamped) by the applicant, who is an authorized signatory in the organization. The applicant is also required to provide the following supporting documents along with the enrollment form: 1 Proof for organization name: a. For companies registered under the Companies Act - Copy of the Certificate of Incorporation (COI) and Memorandum of Association (MOA) attested by a Director of the organization. b. For partnership firms - Copy of the registered partnership deed attested by a partner of the firm. c. For sole proprietorship concerns - A banker’s attestation of the name of the sole proprietorship concern and the signature of the sole proprietor. d. For organizations created by an Act of the Parliament - A copy of the relevant Act or Government Gazette plus an attestation of the application form by an officer of the rank of Head of Department or above. e.
